Completing your ID check is a relatively simple process. Simply choose at least 2 pieces of ID (from section 1 & 2) and visit a Golden Casket shop. Please read the list of ID carefully.

Section 1 Documents
You must provide one of these documents	ID from this list must show your name and date of birth as per the Lotto Direct Player Registration Form.	Passport
		Drivers Licence
		Birth Certificate
		Citizen Certificate (Government issued only)
		18+ Card (Queensland Government issued only)
Section 2 Documents
You must provide one of these documents. It must be in addition to a document from Section 1.	ID from this list must show your name and current Queensland address (PO Box is not accepted). Queensland drivers licence must be valid. All other documents must have been issued within the last 12 months.	Queensland drivers licence
		Electricity, gas or phone account
		Services Notice (rates notice, land tax valuation notice, insurance premium notice, internet service provider statement, or vehicle registration document)
		Letter from your employer certified by a Justice of the Peace, Commissioner for Declarations, Solicitor or Barrister
		Letter or statement from a Tertiary education institution eg TAFE or a university
		Centrelink Pensioner Concession card or Health care card (please ensure the card has not expired)
		Letter from Centrelink
		Residential Tenancy Agreement or rental receipt issued by licensed real estate agent
		Australian Electoral Commission enrolment verification
Section 3 Documents
If no photo ID is shown on ID from Section 1 or 2, you must provide one of these documents, as well as the documents from Sections 1 & 2. ID from this list must show your name as per the Lotto Direct Player Registration Form.		Employer photo ID card

		Student Card with photo from a tertiary education institution

Evidence of Change of Name Documents

If you have changed your name, or your name is different to the details on the document(s) provided, you can provide an Evidence of Change of Name document from the Registrar of Births, Deaths & Marraiges (BDM). Acceptable documents include; a Change of Name Certificate issued by BDM, a Marraige Certificate issued by BDM or an amended Birth Certificate issued by BDM. Divorce papers Decree Nisi or Absolute issued by a court may also be acceptable providing they show the name being reverted to.
